Access without login (Patient without an e-mail address in the patient’s card or new patient)
Without login you have access to:
Entering an order for a doctor's visit - you have a calendar with an overview of dates at your
disposal, you fill in the form and after sending it and processing it with the doctor, the system will
send feedback about the said order to the given e-mail address.
You can delete an order entered in this way no later than one day in advance. You can access the
order via the link in the confirmation e-mail.
After logging in, the following services are available:
1. Entering an order for a doctor's visit - you have a calendar with an overview of dates available,
you fill in the form and after sending it and processing it at the doctor’s, the system will send you
feedback about the said order to your e-mail address.
You can delete an order entered in this way no later than one day in advance.
Overview of order dates - you can see an overview of the dates of your future visits to the
doctor’s office.
2. Sending a message - you can send the doctor information about ongoing treatment or a
question, as well as cancelling a planned visit
Sending a document to be included in the card - you can send a pdf document or an image to
the doctor’s office (e.g. a report from a specialist's examination, etc.)
The doctor can send you quick and brief information.
3. You can track the processing of your requests. You are informed about the processing by e-
mail.
• When accessing the system for the first time, choose "New registration or forgotten password",
fill in your e-mail address, enter consent, and register. The system will guide you through the
following procedure. This will create your login credentials.
• Every time you log in, you can already enter your e-mail address and the selected password
directly in the login screen.

Information for patients (MU employees) / users of SmartMEDIX.net:
The online services of Praktik MUDr. Kolářová s.r.o. are available for our employees to make
appointments for work examinations, related tests and vaccinations.
In order to log in and fully use the services, you must have a patient’s card and be registered.
without registration, you can only use the online services to the extent of placing an order.
• When ordering, please respect the defined types of examination
• If both a work-up and a blood draws are required, please book both appointments at
once. If no appointments are available, you can book the draws and the examination for
another day
• If you are unable to attend a scheduled appointment, please cancel your appointment
yourself at least one day in advance
• If you already have an e-mail address saved in your card, please send the necessary
documents via the online service
Once you enter the login application, you can select the appointment for the examination/blood
collection/vaccination from the available appointments:

After ordering, confirmation will be sent to the subscriber's e-mail address.
When an employee cancels an appointment, confirmation of the cancellation will be sent
to the e-mail address of the subscriber.
