Course objectives
The aim of the course Individual nursing practice is to prepare students for the implementation of diagnostic and therapeutic procedures in intensive care and for the provision of comprehensive, highly specialized nursing care for patients with disabilities of various organ systems.
Learning outcomes
Student after completing the course:
knows the principles of highly specialized nursing care for patients with impaired consciousness and severe impairment of all organ systems and is involved in its provision;
can evaluate the state of consciousness and the depth of analgesic sedation of the patient using evaluation scales;
knows the possibilities of monitoring individual organ systems and uses them; knows the basic complementary examinations and can assess them;
recognizes disorders of the internal environment and participates in their solution;
recognizes heart rhythm disorders on ECG recording and can evaluate it;
is familiar with medicinal products, knows the indications, contraindications, effects, principles of application, applies medicinal products and monitors their effects;
knows the purpose of special instrumentation and uses it;
knows the purpose of devices for UPV implementation, ensures functionality and readiness and uses them;
knows the preparation of the ventilator to the UPV, the procedures of connecting the patient and the principles of patient care to the UPV and participates in the connection of the patient to the UPV;
knows the preparation and connection of the patient to non-invasive artificial lung ventilation and participates in the connection of the patient;
knows the procedures for aspirating the patient with both open and closed aspiration systems and performs aspiration;
knows the principles of nebulizer drug delivery and administers them;
knows the procedures for repositioning the endotracheal cannula and changes its position;
can analyze blood gases and acid-base balance of a patient connected to the UPV and can evaluate data for disconnection from the ventilator;
knows and participates in the procedures of extubation and decanylation of the tracheostomized patient;
knows diagnostic and therapeutic procedures and principles of patient care before, during and after these procedures and participates in procedures and patient care;
can specify the types of nutrition and principles of application to critically ill patients and apply it;
knows the preparation of the patient and the procedure for abdominal and thoracic puncture or drainage, including the care and fixation of the drains, knows the methods of drainage, the principles of pulling the abdominal and thoracic drain and participates in these procedures and care of the patient;
knows the preparation and procedure for gastric lavage in a patient with secured airways and is involved in the performance and care of the patient;
knows and performs the preparation and procedure for male bladder catheterization;
knows and cares about the principles of ostomy care;
knows the effect of immobility on individual organ systems and can prevent changes;
knows and applies the principles of application of kinetic therapy and therapeutic positioning (pronation position, axial rotational therapy) and principles of rehabilitation treatment;
is familiar with documentation and participates in its management;
knows the principles of education and applies them;
can apply psychological approach and communication techniques to critically ill patients and to UPV;
can apply the concept of stimulation of perception;
knows the principles of communication within a multidisciplinary team and applies them;
knows the principles of hygiene and anti-epidemic work regime and applies them;
knows the possibilities of caring for his mental health and applies them.
Student:
accepts the content of the internship, its role and the role of the members of the multidisciplinary team;
is actively involved in all intensive care activities;
accepts and adheres to established rules and procedures;
cooperates with the team;
appreciates the professional and cognitive contribution of practice in intensive care in clinical fields.
Syllabus
  • The subject Individual nursing practice takes place in clinical workplaces of intensive care, specifically in intensive care units of a higher type.
  • List of services that the student performs during the provision of comprehensive, highly specialized nursing care in intensive care units:
  • handling with linear dispenser, infusion pump, enteral pump, ventilator (stable, portable), heating devices, monitor, pulse oximeter, blood analyzer and other devices in intensive care
  • assessment of the state of consciousness using evaluation methodologies and scales
  • evaluation of analgesic sedation depth using evaluation methodologies
  • preparation of a sterile table for aseptic procedures
  • preparation of the fan for UPV
  • connecting the patient to the device to the UPV
  • connecting the patient to non-invasive artificial lung ventilation
  • airway extraction with a closed extraction system
  • airway suction with open suction system
  • application of drugs to the respiratory tract using a nebulizer
  • endotracheal cannula position change
  • monitoring and analysis of ventilation parameters
  • blood sampling for blood gas and acid-base balance in conjunction with UPV and analysis of results
  • evaluation of patient status data when disconnecting from the ventilator weaning the patient from the ventilator
  • patient extubation
  • tracheostomy cannula decanylation
  • comprehensive highly specialized nursing care for patients with impaired consciousness
  • ECG curve analysis
  • catheterization of the male bladder
  • abdominal drain care and drainage system, including abdominal drain removal
  • chest drain care and drainage system, including chest drain extraction
  • stoma care, selection and prescription of devices
  • preventive measures against the consequences of immobilization syndrome, application of specific therapeutic positioning methods
  • application of the concept of stimulation of perception
  • education of the patient and loved ones.
