Course objectives
Introducing the basics of clinical neurological examination with a focus on examining patients with qualitative and quantitative disturbances of consciousness. Fundamentals of neurological topology and syndromology including determination of topical level of disability in coma. Vegetative condition, Locked-in-syndrome, state of minimal consciousness. Neurological disease of a patient in intensive care. Detection and Scoring of Quantitative and Qualitative Disorder of Consciousness (Glasgow Coma Scale [GCS], Richmond Agiation and Sedation Scale [RASS], Confusion Assessment Method in the Intensive Care Unit [CAM / ICU] etc.). The concept of brain death. Basic investigation methods in neurology, focusing on intensive care, their indications, contraindications, complications. Neurological complications of critical condition. Familiarization with basic groups of diseases of the nervous system in childhood and adulthood (stroke, epilepsy, vertebrogenic, neuromuscular, demyelinating, neurodegenerative and metabolic diseases, neurotraumatas, neuroinfectives) focusing on intensive care: theoretical background, overview of current diagnostics and therapy, typical casuistics.
Learning outcomes
The student is able to explain and justify the causes, manifestations, diagnosis, treatment in a patient with severe neurological disease.
Syllabus
  • Pathophysiology of central and neuromuscular complications of critical condition. Systemic Inflammatory Response Syndrome [SIRS], sepsis, multi-organ failure. SOFA scores. Critical state encephalopathy (septic encephalopathy). Delirium: etiopathogenesis, semiology, diagnostics, treatment. Neuromuscular complications of critical condition: polyneuromyopathy of critical condition. Basic principles of nervous system development. Specifics of nervous system affection in childhood age. Fundamentals of neurological semiology (symptomatology). Pulmonary disorders, dyskinesia, myoclonus, speech disorders. Sensory system disorders. Sensory disorders. Vegetative disorders. Clinical neurological examination. Quantitative disturbances of consciousness: somnolence, sopor and coma. Glasgow coma scale. Specific clinical syndromes of a "locked-in-syndrome", state of minimal consciousness, vegetative state. Brain death. Ischemic stroke. Hemorrhagic stroke. Organization of nursing care in the Czech Republic. Neurotraumatology, neuroinfection, degenerative, metabolic and demyelinating disorders of the nervous system in intensive care. Basic investigation methods in neurology. Radiodiagnostic methods (RTG, CT, MR, angiography). Lumbar puncture, examination of liquor. Electro-diagnostic methods (EEG, EMG, EP). Epilepsy in childhood. Cerebral palsy. Epilepsy, status epilepticus. Basics of neurological topology and syndromology. Basic topical neurological syndromes. Topical Diagnosis of Level of Impact of Brain Coma.
