Course objectives
Binocular vision is ability to see with both eyes single image. It isn´t inborn, it develops until one year and is refined until six years. It is divided artificially into 3 grades. Binocular vision is important for the selection of profession and also for social relations and life. Significant defects of single binocular vision (SBV) are strabismus and amblyopia. Strabismus is treated with glasses, occlusion, pleoptics (conservative treatement of amblyopia), surgery and ortoptics (conservative treatment of binocularity). Paralytic strabismus arises from palsy of oculomotor nerves, muscles or defect of central nervous system. Palsy also manifests as a eye´s deviation and consequent SBV defects. Treatment then involves symptomatic procedures (occlusion), spectacle correction, exercise and/or surgical treatment.
Learning outcomes
After this course of lectures/exercise/seminars, student will be able to define a terms pleoptics, ortoptics and divide the single therapeutic procedures according to the applied type of binocular vision dissociation, according to optical conditions (dissociation in the device and real space) and according to their suitability for domestic use. Student will be able to describe the principles and performe practical implementation of pleoptic methods based on the restrictions of the dominant eye and methods that develop (stimulate) vision of amblyopic eye and procedures for normalization of eccentric fixation. Student will know the principles and practical implementations of orthoptic methods focused on the treatment of motor and sensory arm of binocular vision and their mutual synkinetic integration. Student will have a basic knowledge of the history of orthoptics as a distinct subject.
Syllabus
  • In lectures, there is discussed in detail the theoretical basis of the subject (dissociation, excercises in real and device space, excercise load, general principles of dispensing exercise load - impact maximal, long-term submaximal, ...). Rehabilitation of the sensory fusion: reeducation of monocular vision (= pleoptics) (occlusion total, sector, partial, inverse, atropinization, optical penalization, pharmacotherapy, pleoptics active (exercise eye-hand, locator, corrector), passive pleoptics (Campbell, r-TMS, NeuroVision, ..)). Treatment of eccentric fixation (= "passive pleoptics") (pleoptofor by Bangerter, euthyskop and coordinator by Cupers, diffusion filters by Lang, prisms by Pigassou, red filters by Brinker-Katze, ...). Breaking and increase of suppression treshold of dichoptic masking (= "exercise of fusion") (changes in contrast - diffusion and a density neutral filters, changes in position - KRST, changes in brightness- disinhibition, cheiroskop, mirror stereoscope, Brewster-Holmes, reading with grid). Breaking and reduction of suppression threshold of retinal rivalry(diploptics by Avetisov ...). Rehabilitation of motor arm of fusion (orthoptics): Exercise of duction (trainer of eye movements ....). Exercise of relative convergence and divergence in real space (prisms far and near, computer programs, Remy separator, ...) and in the device space (troposkop, Brewster-Holmes, .....). Exercise of convergence within the VAS (push-up, convergence simulator, ...). Normalization of VAS (Remy separator and diploskop).
